Location: Louisville, Kentucky

End Date: 09-21-2026
Contract Type: Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Provide comprehensive physical therapy services including start of care, recertifications, evaluations, and daily visits
- Deliver compassionate care to patients with CHF, orthopedic conditions, and pneumonia in their homes
- Collaborate with the clinical team to develop and implement individualized treatment plans
- Document patient progress and communicate effectively with the interdisciplinary care team
- Support patients on their journey to recovery while working independently in the field
Qualifications
- Current Physical Therapist license in Kentucky
- BLS certification required
- Home health experience preferred but not required
- Strong communication and time management skills
- Ability to work independently while providing excellent patient care
