Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes across Yolo County
- Assess and evaluate each patient's strength, mobility, gait, and rehabilitation potential
- Create and adjust individualized treatment plans in collaboration with physicians
- Educate patients and families on therapeutic exercises and equipment use
- Monitor progress and communicate changes in condition to the care team
- Supervise home health aides and coordinate with other care providers
- Document clinical notes and progress summaries to support quality care
- Participate in ongoing competency assessments and peer consultations
Qualifications
- Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong communication and organizational skills
- Ability to work independently while thriving as part of a collaborative team
- Valid driver's license and reliable transportation for home visits
Knowledge & Skills
- Home health physical therapy expertise
- Patient assessment and rehabilitation planning
- Knowledge of Title 22, Medicare, Medi-Cal, and OSHA regulations
- Clear written and verbal communication
- Comfort working across diverse home environments
