Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM
Hours per Week: 40
Candidate Pay Rate: $56 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes
- Conduct start of care assessments, recertifications, and resumption of care visits
- Deliver 6.5 points of care per day while maintaining excellent patient outcomes
- Collaborate with clinical coordinators and directors for guidance and support
- Complete safety evaluations and teaching visits in hospice situations when needed
- Document patient progress using OASIS standards and best practices
Qualifications
- Current North Dakota Physical Therapy License required
- BLS certification required
- Valid driver's license and auto insurance required
- Home Health experience and OASIS training preferred
- First time travelers welcome to apply
