Roles & Responsibilities

• Evaluate and assess patients' physical therapy needs
• Develop and implement individualized treatment plans
• Guide patients through therapeutic exercises and activities
• Monitor progress and adjust treatments to achieve optimal outcomes
• Educate patients and families on care plans and home exercises
• Collaborate with a multidisciplinary team to ensure comprehensive care

Qualifications

• Active Physical Therapist license in Pennsylvania
• Graduate of an accredited physical therapy program
• Experience with geriatric or memory care populations is a plus
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Commitment to providing compassionate, patient-centered care

Knowledge & Skills

• Knowledge of physical therapy assessment and treatment techniques
• Proficiency in developing and documenting care plans
• Understanding of geriatric conditions and mobility challenges
• Ability to use therapy equipment and modalities
• Strong problem-solving and critical-thinking skills
• Excellent patient education and motivation abilities
