Roles & Responsibilities

• Provide skilled physical therapy treatments to residents in a skilled nursing facility, helping them improve mobility and independence.
• Collaborate with supervising Physical Therapists to implement and adjust individualized treatment plans.
• Document patient progress accurately and communicate effectively with the rehab team and nursing staff.
• Guide residents through therapeutic exercises and activities, offering encouragement and support.
• Maintain a safe, clean therapy environment and follow all infection control and safety protocols.
• Educate residents and their families on home exercise programs and fall prevention.

Qualifications

• Active Physical Therapist Assistant license in the state of Wisconsin (or eligibility to obtain one).
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR/BLS certification.
• Previous experience in skilled nursing or long-term care settings is a plus.
• A compassion for working with older adults and a collaborative, positive attitude.

Knowledge & Skills

• Strong knowledge of therapeutic exercises, mobility training, and modalities.
• Excellent documentation and communication skills.
• Ability to work independently and as part of an interdisciplinary team.
• Time management and organizational skills to manage a caseload effectively.
• Compassion, patience, and a resident-centered approach.
