Roles & Responsibilities

• Conduct comprehensive assessments of residents' physical, cognitive, and psychosocial abilities to develop individualized treatment plans
• Implement therapeutic interventions to improve activities of daily living, mobility, and functional independence
• Collaborate with the interdisciplinary team to ensure coordinated, patient-centered care
• Document patient progress accurately and adjust treatment plans to achieve optimal outcomes
• Educate patients and their families on adaptive techniques and home exercise programs
• Participate in care planning meetings and contribute to discharge planning

Qualifications

• Current Colorado Occupational Therapist license or eligibility to obtain one
• Graduate from an accredited occupational therapy program
• Experience in skilled nursing or long-term care settings preferred, but new grads are welcome to apply
• Excellent communication and interpersonal skills
• Ability to work effectively within an interdisciplinary team
• Strong clinical reasoning and documentation abilities

Knowledge & Skills

• Knowledge of occupational therapy principles, practices, and interventions
• Proficiency in patient assessment and care planning
• Familiarity with electronic health records and documentation systems
• Understanding of Medicare and insurance regulations
• Adaptability to work with diverse patient populations
• Strong organizational and time-management skills
