Roles & Responsibilities

• Conduct comprehensive occupational therapy assessments to identify residents' needs and goals.
• Develop and implement individualized treatment plans that promote independence and enhance daily living skills.
• Collaborate with nurses, physicians, and the care team to ensure holistic resident care.
• Educate residents and families on adaptive techniques, equipment, and home safety to support functional outcomes.
• Maintain accurate documentation of evaluations, progress notes, and discharge summaries in compliance with regulations.

Qualifications

• Must hold a valid Occupational Therapist license in the state of Wisconsin.
• New graduates are encouraged to apply - we value fresh perspectives!
• Prior experience in skilled nursing or rehabilitation is a plus, but not required.
• Excellent communication and interpersonal skills to build rapport with residents and families.

Knowledge & Skills

• Strong clinical reasoning and assessment abilities.
• Knowledge of occupational therapy interventions and activity analysis.
• Ability to work effectively within a multidisciplinary team.
• Proficiency in electronic documentation systems (or ability to learn).
• Compassionate and patient-centered approach to care.
