Occupational Therapist - Skilled Nursing Facility
Location: Laconia, NH
Start Date: 08-31-2026
Contract Type: Perm
Shift Type: Monday-Friday, 8:00 AM - 4:30 PM; weekend rotation as required
Hours per Week: 40
Bill Rate: $37-$43/hour
Benefits

Roles & Responsibilities
- Evaluate and treat residents with physical, cognitive, or emotional limitations to help them achieve maximum independence
- Develop and implement individualized treatment plans tailored to each resident's goals
- Collaborate with the interdisciplinary care team to ensure comprehensive rehabilitation services
- Document resident progress and adjust treatment plans to optimize functional outcomes
- Educate residents and families on therapeutic techniques and adaptive strategies
- Participate in discharge planning and coordinate care for successful transitions
Qualifications
- Active Occupational Therapist (OT) license in New Hampshire or eligibility to obtain
- Master's or Doctoral degree in Occupational Therapy from an accredited program
- Previous experience in skilled nursing, rehabilitation, or long-term care settings preferred
- Strong clinical assessment, treatment planning, and documentation skills
- Excellent communication and teamwork abilities
- A genuine passion for helping older adults and individuals with complex medical needs
Knowledge & Skills
- In-depth knowledge of occupational therapy assessments and evidence-based interventions
- Ability to adapt treatment plans to meet individual resident needs
- Understanding of Medicare/Medicaid documentation and billing requirements
- Proficiency with electronic health records (EHR) systems
- Strong problem-solving, critical thinking, and organizational skills
- Compassionate, patient-centered approach to care
