Roles & Responsibilities

• Evaluate and treat residents with physical, cognitive, or emotional limitations to help them achieve maximum independence
• Develop and implement individualized treatment plans tailored to each resident's goals
• Collaborate with the interdisciplinary care team to ensure comprehensive rehabilitation services
• Document resident progress and adjust treatment plans to optimize functional outcomes
• Educate residents and families on therapeutic techniques and adaptive strategies
• Participate in discharge planning and coordinate care for successful transitions

Qualifications

• Active Occupational Therapist (OT) license in New Hampshire or eligibility to obtain
• Master's or Doctoral degree in Occupational Therapy from an accredited program
• Previous experience in skilled nursing, rehabilitation, or long-term care settings preferred
• Strong clinical assessment, treatment planning, and documentation skills
• Excellent communication and teamwork abilities
• A genuine passion for helping older adults and individuals with complex medical needs

Knowledge & Skills

• In-depth knowledge of occupational therapy assessments and evidence-based interventions
• Ability to adapt treatment plans to meet individual resident needs
• Understanding of Medicare/Medicaid documentation and billing requirements
• Proficiency with electronic health records (EHR) systems
• Strong problem-solving, critical thinking, and organizational skills
• Compassionate, patient-centered approach to care
