Roles & Responsibilities

• Coordinate comprehensive patient care from admission through discharge
• Collaborate with interdisciplinary teams to develop personalized care plans
• Monitor patient progress and adjust plans to ensure optimal outcomes
• Educate patients and families on healthcare options and resources
• Advocate for patient needs and ensure smooth transitions of care

Qualifications

• Active Registered Nurse (RN) license in Massachusetts
• Strong clinical assessment and critical thinking skills
• Excellent communication and interpersonal abilities
• Ability to work independently and as part of a team
• Prior case management experience preferred but not required

Knowledge & Skills

• Knowledge of discharge planning and utilization management
• Familiarity with electronic health records (EHR) systems
• Understanding of healthcare regulations and reimbursement models
• Proficiency in Microsoft Office applications
