Roles & Responsibilities

• Coordinate patient care plans from admission to discharge
• Collaborate with physicians, nurses, and social workers to ensure optimal outcomes
• Assess patient needs and advocate for appropriate resources
• Facilitate timely and safe discharge planning
• Document and communicate care plans effectively

Qualifications

• Active Registered Nurse (RN) license in Massachusetts
• Basic Life Support (BLS) certification required
• Experience in case management or discharge planning preferred
• Strong communication and organizational skills
• Ability to work effectively in a fast-paced healthcare setting

Knowledge & Skills

• Critical thinking and problem-solving abilities
• Patient advocacy and interdisciplinary collaboration
• Understanding of healthcare regulations and reimbursement
• Proficiency in electronic health records (EHR) systems
