Roles & Responsibilities

• Provide compassionate, patient-centered care to residents in a transitional rehab center
• Assist patients with activities of daily living including bathing, dressing, grooming, and toileting
• Monitor and record vital signs, reporting any changes to the nursing team
• Help patients with mobility, transfers, and repositioning to ensure safety and comfort
• Maintain a clean, organized, and safe environment for patients and staff
• Document care provided accurately and promptly in the EMR

Qualifications

• Current Certified Nursing Assistant (CNA) certification in New Mexico
• BLS/CPR certification required
• COVID-19 vaccination or approved medical/religious exemption
• Must pass background check and provide government-issued ID
• Prior experience in rehab, long-term care, or acute care is a plus
• Strong communication and teamwork skills

Knowledge & Skills

• Knowledge of patient care techniques and safety protocols
• Understanding of HIPAA and patient confidentiality
• Ability to use electronic health records and care documentation systems
• Physical ability to lift, transfer, and support patients
• Compassionate, patient, and empathetic approach to care
• Strong attention to detail and ability to multitask
