Contract Type: Travel Contract

Shift Type: 40 hours per week, day shifts, rotating weekends (2-3 per month), on-call required (6 shifts per month including weekends)

Roles & Responsibilities

Coordinate patient care across interdisciplinary teams to ensure optimal outcomes
Serve as a liaison between patients, families, physicians, and healthcare beneficiaries
Develop, implement, and revise case management plans for complex patients
Monitor resource utilization and appropriateness of care to optimize patient outcomes
Manage frequent emergency department visitors with focus studies to improve care coordination

Qualifications

At least 1 year of experience in a Medicare-certified home health agency (required)
Valid driver's license and personal car insurance (required)
Current Hawaii state license (required)
Proficiency with Homecare Home Base and OASIS (required)
COVID-19 and Flu vaccination (required, no exceptions)
Bachelor's degree in nursing or related field (preferred)
