Roles & Responsibilities

• Provide direct, compassionate patient care in accordance with nursing standards
• Administer medications and treatments safely and document appropriately
• Monitor and record vital signs, recognizing changes in patient condition
• Collaborate closely with RNs, physicians and interdisciplinary team members
• Maintain clear, timely charting and handoffs to ensure continuity of care
• Assist with patient mobility, ADLs and maintaining a safe environment
• Educate patients and families about care plans, medications and follow-up

Qualifications

• Current LPN license in New Hampshire or eligibility to obtain state licensure
• Basic Life Support (BLS) certification or willingness to obtain upon hire
• Current immunizations and ability to complete required health screenings
• Comfortable with electronic medical records and accurate documentation
• Reliable, patient-centered, and able to work independently while contributing to a team
• Ability to pass background check and any facility-required screenings

Education

• Completion of an accredited Practical Nursing program or equivalent diploma/certificate

Knowledge & Skills

• Strong fundamentals in medication administration and clinical assessments
• Skilled at vital signs, basic wound care and activities of daily living support
• Clear communication with patients, families and clinical teams
• Good time management, prioritization and attention to detail
• Familiarity with infection control practices and patient safety principles
