Contract Type: Travel Contract

Shift Type: Monday-Friday, 7:00 AM - 3:30 PM

Hours per Week: 40

Candidate Pay Rate: $42 per hour

Roles & Responsibilities

Provide compassionate, hands-on care to residents in a supportive rehab setting
Administer medications and monitor patient responses with attention to detail
Collaborate with the care team to create and update individualized care plans
Support daily living activities and help residents maintain their independence
Document care accurately and communicate changes in condition promptly

Qualifications

Active LPN license in Vermont or compact state
BLS certification required
COVID-19 vaccination or approved exemption
Reliable transportation for every-other-weekend coverage
Strong clinical skills and a caring bedside manner

Knowledge & Skills

Medication administration and patient monitoring
Electronic health record documentation
Infection control and safety protocols
Patient education and family communication
Team collaboration in a rehab environment
