Location: Grand Junction, Colorado

Start Date: 07-27-2026

Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, with occasional weekend and on-call backup

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide compassionate, individualized hospice care to patients in their homes, nursing homes, group homes, or assisted living facilities
Lead the interdisciplinary team as the primary RN case manager, coordinating care plans and symptom management
Educate patients and families on disease processes, goals of care, and caregiving techniques
Conduct comprehensive clinical assessments and use critical thinking to adapt care plans
Support patients and families with strength and courage during end-of-life transitions

Qualifications

Current, active RN license in Colorado (compact license accepted)
Minimum 2 years of nursing experience, preferably in hospice or home health
Strong clinical assessment and critical thinking skills
A passion for teaching and empowering patients and families

Knowledge & Skills

Hospice nursing and end-of-life care
Symptom management across disease processes
Patient education and family support
Clinical assessment and care planning
Interdisciplinary team collaboration
