Contract Type: Travel Contract

Shift Type: 8-hour day shifts with weekend rotation, typically 8:00 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $46 per hour

Roles & Responsibilities

Manage a caseload of roughly 30 adult patients in their homes
Perform Start of Care assessments, recertifications, and daily visits
Coordinate patient care plans and collaborate with interdisciplinary teams
Document patient progress using Homecare Homebase (HCHB) system
Educate patients and families on health management and home safety
Provide skilled nursing care following physician orders and clinical guidelines

Qualifications

Active RN license in Kentucky (or compact) required
Minimum 1 year of Med-Surg experience in a hospital setting
At least 1 year of Home Health experience mandatory
Proficiency with Homecare Homebase (HCHB) system required
Current BLS certification from AHA required
Valid driver's license and reliable transportation (no mileage reimbursement)

Knowledge & Skills

Strong clinical assessment and critical thinking skills
Excellent time management and organizational abilities
Comfort with independent decision-making in the field
Proficient in electronic health records, specifically HCHB
Exceptional communication and patient education skills
