Candidate Pay Rate: $49 per hour

Roles & Responsibilities

Deliver compassionate care to patients across a variety of diagnoses including cardio, pulmonary, orthopedic, neurological, and geriatric conditions
Provide personalized physical therapy treatments in patients' homes throughout the greater Sacramento county area
Collaborate with interdisciplinary teams to create and implement effective care plans
Track and document patient progress while meeting productivity goals of 5-6 patients per day
Build meaningful relationships with patients and their families while supporting their recovery journey

Qualifications

Active Physical Therapy Assistant license in California
Minimum experience requirements vary - new grads considered in extreme cases only
Strong clinical skills across multiple specialty areas
Excellent communication and time management abilities

Knowledge & Skills

Home health care experience preferred
Ability to work independently in various home settings
Strong documentation and organizational skills
Comfortable treating diverse patient populations
Reliable transportation for home visits
