Roles & Responsibilities

• Assess clients' conditions and develop individualized care plans to maximize mobility and independence
• Collaborate with an interdisciplinary team to ensure comprehensive care
• Perform standard therapy measurements including ROM, strength, and balance tests
• Utilize a variety of therapeutic modalities to enhance client recovery
• Manage cases for home health clients, ensuring continuity and quality of care

Qualifications

• Active Vermont Physical Therapist license
• Minimum 1 year of home health experience
• OASIS experience required
• Experience with McKesson, Netsmart, or Homecare Homebase EMR preferred
• Certifications: ACLS, BLS, PALS, NRP, ASHA, CHT, ACP Training
