Roles & Responsibilities

Provide compassionate physical therapy services across four rural counties
Conduct Oasis Assessments and Start of Care evaluations for new patients
Develop personalized treatment plans for elderly patients recovering from falls and fractures
Track visits and time using provided tablet technology
Collaborate with local healthcare teams to deliver exceptional patient outcomes

Qualifications

Current Physical Therapist license in Indiana
Experience with Oasis Assessments and Start of Care documentation
Comfortable working independently in rural settings
Strong communication and time-management skills
