Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Provide personalized physical therapy services to 5-7 patients daily in their homes
Help patients recover from surgeries, falls, and other conditions with compassionate care
Document patient progress using the HomeCare/HomeBase EMR system
Travel within a 60-mile radius to deliver high-quality home health therapy
Collaborate with interdisciplinary teams to support Home Health and Hospice patients

Qualifications

Current North Dakota Physical Therapy License required
BLS certification needed
At least 1 year of previous home health experience
Valid driver's license and auto insurance required
Strong PT-related clinical skills

Knowledge & Skills

Home health physical therapy expertise
Comfortable working independently in patient homes
Familiarity with HomeCare/HomeBase EMR
Excellent time management and organizational abilities
Compassionate approach to patient-centered care
