Roles & Responsibilities

• Provide compassionate physical therapy services to patients in their homes
• Develop and implement individualized care plans focused on improving mobility and independence
• Collaborate with the interdisciplinary care team to ensure holistic patient care
• Complete OASIS documentation accurately and in a timely manner
• Educate patients and families on home exercise programs and safety strategies
• Adapt treatment approaches to meet the unique needs of each patient

Qualifications

• Active Physical Therapist license in California
• Current BLS certification
• Minimum of 2 years of experience as a Physical Therapist
• Experience with OASIS documentation required
• Home health experience preferred
• Familiarity with Cerner EMR preferred

Knowledge & Skills

• Strong clinical knowledge of orthopedic, stroke, joint, and spinal injury rehabilitation
• Ability to work independently in a home-based setting
• Excellent communication and patient education skills
• Solid documentation skills with attention to detail
• Time management skills to handle a flexible daily schedule
• Compassionate approach to patient care
