Contract Type: Contract

Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with every 3rd weekend rotation

Hours per Week: 40

Candidate Pay Rate: $64 per hour

Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across assigned geographic regions
Evaluate and treat a diverse caseload including orthopedic conditions, strokes, joint replacements, and spinal injuries
Complete OASIS documentation accurately as required for government compliance
Collaborate with OT and Speech colleagues to provide comprehensive home-based rehabilitation
Support patients and families with education and care planning for optimal recovery
Maintain professional business attire with white lab coat while representing UHC in the community

Qualifications

Active Physical Therapist license in California
Minimum one year of experience working in PT, OT, or Speech
Experience with OASIS documentation required
BLS certification required
Home health setting experience preferred but not required
Cerner EMR experience preferred

Knowledge & Skills

Strong assessment and treatment planning skills for home health patients
Proficiency with OASIS and Cerner EMR documentation
Ability to work independently while collaborating with interdisciplinary teams
Excellent communication and patient education abilities
Flexible and adaptable approach to scheduling and patient needs
