Physical Therapist - Home Health
Location: Louisville, KY
Start Date: 06-29-2026
End Date: 09-28-2026
Contract Type: Travel Contract
Shift Type: Rotating schedule including weekends, 8:00 AM - 5:00 PM (5x8-hour shifts)
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Conduct comprehensive start-of-care, recertifications, and evaluations for patients in a home health setting
- Develop and implement individualized treatment plans to improve patient mobility and function
- Collaborate with a multidisciplinary team and communicate progress with patients and families
- Document patient care accurately and timely using provided tablet and phone
- Provide patient education and home exercise programs to promote independence
Qualifications
- Active Physical Therapist license in Kentucky or compact state
- BLS certification (required)
- Preference for candidates with home health experience; new grads considered
- Ability to work rotating weekends and holidays
Education
- Doctorate or Master's degree in Physical Therapy from an accredited program
Knowledge & Skills
- Strong clinical assessment and critical thinking skills
- Knowledge of home health regulations and OASIS documentation
- Excellent communication and patient education abilities
- Ability to work independently and manage a daily caseload
