Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM

Hours per Week: 40

Candidate Pay Rate: $61 per hour

Roles & Responsibilities

Plan and lead treatment programs that restore and improve physical, social, and mental function
Conduct thorough evaluations to identify patient needs and set meaningful goals
Design individualized treatment plans using evidence-based techniques
Deliver hands-on therapy interventions that help patients regain independence
Coordinate care and discharge planning with the full interdisciplinary team
Maintain accurate, timely clinical documentation that supports quality care
Mentor and supervise therapy assistants to deliver excellent outcomes

Qualifications

Current Hawaii state Physical Therapist license
Minimum 2 years of Physical Therapy experience
At least 1 year of home health PT experience
Proficiency with Homecare Home Base and OASIS documentation
Bachelor's degree in Physical Therapy (Master's preferred)
Travel experience is a plus

Education

Bachelor's degree in Physical Therapy required
Master's degree preferred

Knowledge & Skills

Strong patient evaluation and assessment skills
Ability to create individualized, goal-oriented treatment plans
Comfortable using clinical documentation systems
Excellent communication and teamwork abilities
Mileage reimbursement at 43 cents per mile
