Roles & Responsibilities

• Conduct physical therapy assessments and treatments for home health patients
• Develop and implement individualized treatment plans
• Collaborate with other healthcare professionals to ensure comprehensive care
• Provide education and training to patients and caregivers
• Maintain accurate records and documentation

Qualifications

• Current ND Physical Therapy License
• BLS certification
• 1 year of experience in physical therapy
• Home health experience preferred
• OASIS training preferred

Education

• Physical Therapy degree from an accredited program

Knowledge & Skills

• Physical therapy techniques and modalities
• Home health care principles and practices
• OASIS data collection and reporting
• Communication and interpersonal skills
• Time management and organization
