Roles & Responsibilities

• Provide comprehensive physical therapy services to patients in their homes
• Develop individualized treatment plans to improve mobility and function
• Educate patients and families on therapeutic exercises and techniques
• Collaborate with interdisciplinary team to ensure coordinated care
• Document patient progress accurately and timely
• Adapt treatment strategies to meet patient goals and enhance quality of life

Qualifications

• Valid Physical Therapy license in Iowa required
• BLS certification required
• Home health experience preferred
• Ability to work independently and manage a caseload
• Excellent communication and interpersonal skills
• Flexibility to travel to patient homes within the service area

Knowledge & Skills

• Knowledge of physical therapy principles and practices
• Skilled in patient assessment and treatment planning
• Proficiency with Epic preferred, but not required
• Strong documentation and time management skills
• Ability to build rapport with patients and families
