Roles & Responsibilities

• Provide skilled occupational therapy services to residents in a skilled nursing rehabilitation setting
• Assess functional status and perform thorough evaluations to guide treatment planning
• Develop and implement individualized treatment plans to improve activities of daily living (ADLs)
• Document progress thoroughly and communicate clinical changes clearly to the care team
• Collaborate with nurses, PTs, SLPs and other team members to ensure coordinated, patient-centered care
• Educate residents and families on strategies to maximize independence and safety at home

Qualifications

• Licensed Occupational Therapist (state license required; Michigan licensure preferred or eligible)
• Registered Occupational Therapist (OTR) credential or equivalent
• Experience in skilled nursing facility (SNF) or rehabilitation settings preferred
• Current CPR/BLS certification (or willingness to obtain prior to start)
• Strong clinical documentation and time-management skills
• Excellent communication skills and a team-oriented approach

Education

• Degree in Occupational Therapy (Bachelor's, Master's, or OTD as required by state licensure)

Knowledge & Skills

• Skilled in ADL retraining, adaptive equipment recommendations, and therapeutic interventions
• Knowledgeable in fall prevention, transfer training and safe mobility techniques
• Comfortable with interdisciplinary care planning and clinical documentation systems
• Ability to evaluate and modify treatment based on resident progress and goals
• Strong patient-education skills and family/caregiver coaching
