Roles & Responsibilities

Provide compassionate direct patient care and support daily living needs
Administer medications and treatments accurately and on time
Monitor and document vital signs, symptoms, and clinical changes
Collaborate with nurses, physicians, and care team members to deliver excellent care
Educate residents and families about care plans and health promotion
Maintain accurate and timely clinical records and handoffs
Follow infection control and safety protocols to protect residents and staff

Qualifications

Current Licensed Practical Nurse (LPN) license in Pennsylvania required
Active Basic Life Support (BLS) or CPR certification required or obtainable upon hire
Previous experience in long term care, skilled nursing, or similar setting is preferred
Strong communication and interpersonal skills with a resident-centered attitude
Ability to pass background checks and meet facility onboarding requirements

Education

Graduate of an accredited practical nursing program or equivalent certificate/diploma

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and pressure injury prevention
Familiarity with electronic health records and clinical charting
Ability to assess and report changes in patient condition promptly
Time management and prioritization in a fast-paced care environment
Teamwork and collaboration with multidisciplinary staff
