Roles & Responsibilities

Provide compassionate, professional nursing care to walk-in clinic patients following established standards and scope of practice
Serve as the primary liaison between patients, clinic staff, and providers to ensure clear, timely communication
Take and document vital signs and assess patient condition to support effective triage
Triage patient calls and determine appropriate next steps or escalate to providers when needed
Process medication refills and complete necessary prior authorizations to keep patients on their therapies
Support and collaborate with the interdisciplinary care team to meet patient needs and clinic goals
Provide direction to CNAs and Schedulers and work under the guidance of Providers, Coordinators, Clinical Managers/Supervisors, and RNs
Maintain licensure and follow continuing education requirements to deliver safe, up-to-date care

Qualifications

Graduate of an accredited Nursing Program
Current State LPN license (must be active and in good standing)
Current Basic Life Support (BLS) certification
Comfortable providing triage, taking vital signs, and managing medication refill and prior authorization workflows
Excellent communication skills and ability to work collaboratively with clinical teams

Education

Graduate of a Nursing Program (LPN)

Knowledge & Skills

Licensed Practical Nurse (LPN) clinical skills
Vital signs measurement and assessment
Triage and patient prioritization
Medication refill processing and prior authorization procedures
Basic Life Support (BLS) proficiency
Strong patient communication and care coordination
