Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
Administer medications and monitor vital signs with attention to detail
Collaborate with the care team to support individualized resident plans
Document care accurately and communicate changes in condition promptly
Create a warm, supportive environment that residents and families can trust

Qualifications

Active LPN license in West Virginia
Current CPR certification
Strong clinical skills and attention to detail
Ability to work independently and as part of a team
COVID-19 vaccination or approved exemption required

Knowledge & Skills

Skilled nursing and long-term care experience
Medication administration and documentation
Patient assessment and vital signs monitoring
Clear communication with residents, families, and care teams
Flexibility to work night shifts and rotating weekends
