Roles & Responsibilities
- Provide safe, compassionate patient care and monitor vital signs
- Administer medications and treatments per plan of care
- Document patient status and communicate changes to the care team
- Assist with activities of daily living to promote patient comfort
- Collaborate with nurses and interdisciplinary staff to support positive outcomes
- Maintain patient confidentiality and adhere to facility policies
Qualifications
- Active LPN license or eligible for licensure in Pennsylvania
- Current CPR/BLS certification (or willingness to obtain)
- Comfortable working focused 4-hour shifts and able to provide reliable coverage
- Strong communication skills and a patient-centered attitude
- Previous experience in long-term care or related settings is a plus
Education
- Completion of an accredited Licensed Practical Nurse program
- High school diploma or GED
Knowledge & Skills
- Medication administration and basic wound care
- Patient assessment and vital sign monitoring
- Electronic charting and accurate documentation
- Infection control and standard precautions
- Teamwork, time management, and prioritization in a fast-paced environment
- Compassionate patient and family communication
