Shift Type: Monday-Friday, some weekends as needed; hours vary (primarily day shifts)

Roles & Responsibilities

Provide high-quality nursing care to clinic patients, following established standards and your LPN scope of practice.
Serve as the primary liaison between patients, providers, and clinic staff to ensure smooth communication and continuity of care.
Take and interpret vital signs and perform bedside assessments to support clinical decision-making.
Triage patient calls, determine urgency, and coordinate next steps with providers and the care team.
Process medication refills and complete prior authorizations accurately and promptly.
Support and direct CNAs and Schedulers in daily workflows to keep the clinic efficient and patient-centered.
Collaborate with RNs, Providers, Coordinators, and Clinical Managers to meet patient needs and improve care delivery.

Qualifications

Graduate of an accredited nursing program (LPN)
Current State LPN license required
Basic Life Support (BLS) certification required
Comfortable triaging calls and managing vital signs and clinical assessments
Experience with medication refills, prior authorizations, and care coordination preferred
Strong communication skills and ability to provide direction to CNAs and Schedulers

Education

Graduate of a Nursing Program

Knowledge & Skills

Clinical nursing fundamentals and outpatient/urgent care workflows
Triage protocols and effective patient assessment
Taking and interpreting vital signs
Medication refill processes and prior authorization workflows
Care coordination and team-based communication
Supervisory direction for support staff (CNAs, Schedulers)
