Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications and treatments safely and on time
Monitor patient condition, record vital signs, and report changes promptly
Document care accurately in the charting system to ensure continuity
Collaborate with RNs, providers, and interdisciplinary teams to optimize care
Support admissions, discharges, and transitions with clear communication
Promote a safe, clean environment and follow infection control practices

Qualifications

Valid Licensed Practical Nurse (LPN) license (NH license or ability to obtain)
Current BLS/CPR certification (or willingness to obtain upon hire)
Comfortable and experienced working night shifts
Strong communication skills and a team-oriented attitude
Experience in long-term care or similar settings preferred but not required

Education

Graduation from an accredited Practical Nursing program or equivalent diploma

Knowledge & Skills

Medication administration and monitoring
Basic wound care and dressing changes
Patient assessment and documentation
Familiarity with electronic health record charting
Infection control practices and safety procedures
Ability to assist with activities of daily living and mobility support
