Contract Type: Per-Diem

Shift Type: Per diem (days vary), 6:00 AM - 6:30 PM (day)

Hours per Week: 38

Candidate Pay Rate: $30 per hour

Roles & Responsibilities

Provide compassionate, patient-centered nursing care to residents in a rehab setting
Administer medications and treatments safely and accurately
Assess patient condition and report changes to the interdisciplinary team
Support rehabilitation activities and assist with mobility and ADLs
Maintain clear, timely documentation in the medical record
Collaborate with RNs, therapists, and providers to coordinate care plans
Ensure a safe environment by following infection control and safety protocols

Qualifications

Current LPN licensure (Licensed Practical Nurse) — New Mexico state licensure or equivalent required
Active clinical skills in medication administration and basic nursing care
Valid BLS/CPR certification (or willing to obtain upon hire)
Comfort working in a rehab/long-term care environment and supporting mobility goals
Strong communication skills and the ability to work well on a team

Education

Graduate of an accredited LPN program
For RN candidates (if applicable): ADN or BSN from an accredited nursing program

Knowledge & Skills

Skilled in patient assessment and basic nursing interventions
Proficient with medication administration and documentation
Familiarity with rehab-focused care and assisting with ADLs
Effective communication with patients, families, and clinical teams
Detail-oriented with strong charting and time-management abilities
