Roles & Responsibilities
- Conduct thorough psychosocial assessments to identify patient needs and risks across the lifespan
- Develop individualized, evidence-based treatment plans and adjust them as patients progress
- Deliver therapeutic interventions (individual, family, group) for depression, anxiety, trauma, substance use, and adjustment disorders
- Collaborate closely with primary care providers, nurses, and behavioral health colleagues to support integrated, whole-person care
- Maintain accurate, timely clinical documentation and ensure compliance with agency and regulatory standards
- Advocate for underserved individuals and families, connecting them with community resources and care coordination supports
Qualifications
- Active LISW license in Iowa (required)
- Master’s degree in Social Work from an accredited program
- Minimum of 2 years of clinical experience preferred
- Experience working in rural or medically underserved settings is a plus
- Strong interpersonal, organizational, and clinical documentation skills
Education
- Master’s degree in Social Work from an accredited program (required)
Knowledge & Skills
- Skilled in psychosocial assessment and treatment planning
- Experience delivering evidence-based therapeutic interventions
- Comfortable working on an integrated care team and coordinating with primary care
- Ability to address a broad range of behavioral health issues including trauma and substance use
- Professional documentation practices and electronic health record familiarity
