Location: Red Bluff, California

Shift Type: Tuesday-Saturday, 8:00 AM - 4:00 PM

Hours per Week: 40

Candidate Pay Rate: $110 - $77 per hour

Roles & Responsibilities

Provide skilled nursing care to recently discharged patients in their homes
Educate patients and families on managing CHF, COPD, wounds, and IV-PICC lines
Complete start-of-care, recertification, resumption-of-care, and routine visits
Document visits accurately using HomeCare HomeBase
Collaborate with home health aides and the interdisciplinary team
Help reduce hospital readmissions through proactive patient support

Qualifications

Active Registered Nurse license in California
BLS certification
Valid driver's license and auto insurance
Home Health experience preferred
HomeCare HomeBase experience a plus

Knowledge & Skills

Skilled in wound care and wound vac management
Comfortable with IV-PICC line care and patient education
Strong assessment and documentation abilities
Ability to manage a 5-visit daily caseload
Flexible and adaptable to schedule changes
