Roles & Responsibilities

• Evaluate patients and develop personalized treatment plans based on physician orders and referrals
• Deliver skilled occupational therapy services to help patients improve independence in daily living
• Create and update individualized treatment and discharge plans as patients progress
• Document all services accurately and in a timely manner
• Supervise Certified Occupational Therapy Assistants (COTAs) in line with practice acts and facility policies
• Collaborate with interdisciplinary care teams and families to ensure holistic patient care
• Maintain compliance with all safety, regulatory, and facility standards

Qualifications

• Graduate of an accredited Occupational Therapy program (MOT or OTD)
• Active state Occupational Therapy license, or the ability to obtain one
• Strong clinical, documentation, and communication skills
• Previous experience in skilled nursing or rehabilitation settings is a plus
