Roles & Responsibilities

• Provide high-quality nursing care to residents in a skilled nursing facility
• Administer medications and treatments as prescribed by physicians
• Monitor patient vital signs and report changes to the healthcare team
• Assist with activities of daily living and ensure patient comfort
• Collaborate with interdisciplinary team to develop and implement care plans
• Document patient progress and maintain accurate medical records

Qualifications

• Active LPN license in West Virginia or compact state
• Minimum 1 year of experience in long-term care or skilled nursing preferred
• Current BLS certification required
• Ability to work night shifts and rotating weekends
• Strong communication and teamwork skills

Knowledge & Skills

• Proficient in medication administration and wound care
• Knowledge of EHR systems and documentation standards
• Excellent assessment and critical thinking abilities
• Compassionate patient-centered approach
• Ability to adapt to fast-paced environment
