Location: Prescott, Arizona

Start Date: 06-08-2026

End Date: 09-07-2026

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM, with possible weekends every 3-4 weeks

Hours per Week: 40

Candidate Pay Rate: $50 per hour

Roles & Responsibilities

Complete SW assessments
Conduct Mental Health assessments
Title 36 petitions as needed
DC planning as needed
Address homelessness and substance abuse
SDOH consults and maternal child assessments
DCS and APS reporting as needed

Qualifications

LMSW or LCSW required
Experience in Social Work
Cerner experience preferred

Education

Social Work degree required

Knowledge & Skills
