Candidate Pay Rate: $74 per hour

Roles & Responsibilities

Provide compassionate clinical social work services to patients and families
Conduct thorough psychosocial assessments and develop individualized care plans
Coordinate care across multidisciplinary teams to ensure seamless patient support
Facilitate 5150 evaluations and involuntary psychiatric holds when clinically appropriate
Connect patients with vital community resources and support services
Advocate for patients navigating complex healthcare and social systems

Qualifications

Active California LCSW license required
Current BLS certification
5150 certification in California required
Prior experience with Dignity Health facilities strongly preferred
Strong clinical assessment and crisis intervention skills

Knowledge & Skills

Expertise in involuntary psychiatric evaluation processes
Knowledge of California mental health laws and regulations
Strong communication and documentation abilities
Ability to work collaboratively within interdisciplinary teams
Understanding of care coordination best practices
