Roles & Responsibilities

Deliver personalized physical therapy services in patients' homes across Yolo County
Assess and evaluate each patient's therapeutic needs and create tailored care plans
Guide patients and families on using therapeutic equipment and exercises
Monitor progress and communicate updates to physicians and care teams
Train and supervise home health aides participating in rehabilitation programs
Document clinical notes and progress summaries to support quality care
Collaborate in case conferences to ensure coordinated patient outcomes

Qualifications

Minimum 2 years of experience as a Physical Therapist in acute, home care, or rehab settings
Current BLS certification required
Strong communication and organizational skills
Valid driver's license and reliable transportation for home visits

Knowledge & Skills

Physical Therapy assessment and treatment planning
Home environment evaluation and patient mobility analysis
Patient and caregiver education techniques
Documentation and clinical reporting best practices
Understanding of Medicare, Medi-Cal, and Title 22 regulations
