Roles & Responsibilities

Plan and execute personalized rehabilitation programs that help patients regain strength and mobility
Develop comprehensive Plans of Care with short and long-term goals tailored to each patient
Evaluate mobility, strength, and function while setting realistic, attainable improvement targets
Implement therapeutic exercise, gait training, manual therapy, and neuromuscular re-education techniques
Document patient progress accurately and maintain compliance with payer requirements
Supervise physical therapy assistants with professionalism and consistency
Collaborate with facility staff and referral sources to deliver seamless, compassionate care
Create meaningful home exercise programs and discharge instructions for lasting results

Qualifications

Active Illinois Physical Therapist License required
Current American Heart Association BLS certification required
Minimum 1 year of Physical Therapy experience required
Outpatient experience strongly preferred
Strong documentation and communication skills

Knowledge & Skills

Therapeutic Exercise and Gait Training expertise
Manual Therapy techniques
Plan of Care development and patient evaluation
Anatomy, biomechanics, and exercise physiology knowledge
Clear, compassionate patient communication
Accurate medical documentation practices
