Roles & Responsibilities

Treat patients with movement dysfunction or pain to boost their ability to perform daily tasks and enhance independence
Decrease fall risk and adverse events while improving mobility and functional outcomes
Assist in recovery from surgery and support discharge planning for better patient results
Document evaluations, progress notes, discharges, and all patient care details accurately
Communicate effectively with providers, insurance companies, patients, and rehab team
Help maintain a clean and welcoming treatment environment

Qualifications

Licensed Physical Therapist in Wisconsin
Experience in outpatient or rehab settings preferred
Strong passion for patient-centered care

Education

Doctor of Physical Therapy (DPT) or Master's in Physical Therapy
Current state licensure required

Knowledge & Skills

Expertise in pain management and mobility improvement
Skilled in fall risk reduction and discharge planning
Excellent documentation and communication abilities
Team-oriented with attention to detail
