Roles & Responsibilities

• Provide physical therapy services to clients in home health settings
• Collaborate with team members to develop and implement comprehensive plans of care
• Conduct patient assessments and evaluate progress toward goals
• Utilize rehabilitation equipment including splints, orthoses, prostheses, and wheelchairs
• Document patient care and maintain accurate records using OASIS

Qualifications

• Active Massachusetts Physical Therapist license (required)
• BLS certification through American Heart Association (required)
• Minimum 2 years of acute nursing care and home care experience (required)
• Valid driver's license and reliable vehicle (required)
• Home Health Certified with proficiency in OASIS (required and non-negotiable)
• Ability to work independently with minimal supervision

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Proficient in OASIS documentation and home health regulations
• Excellent communication and interpersonal abilities
• Experience with rehabilitation equipment and modalities
• Time management and self-direction in a home care environment
