Location: Louisville, Kentucky

End Date: 09-21-2026
Contract Type: Travel Contract
Shift Type: Monday-Friday, 8:00 AM - 5:00 PM with rotating weekend shifts
Hours per Week: 40
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Provide compassionate physical therapy services in patients' homes
- Conduct start of care assessments, recertifications, and evaluations
- Develop personalized treatment plans for patients with CHF, orthopedic conditions, and pneumonia
- Collaborate with clinical managers and educators to ensure excellent patient outcomes
- Document care and communicate effectively with the healthcare team
Qualifications
- Active Physical Therapist license in Kentucky
- BLS certification required
- Home health experience preferred but not required
- No new grads please
Knowledge & Skills
- Home health physical therapy expertise
- Strong assessment and evaluation skills
- Excellent communication and documentation abilities
- Ability to work independently in the field
