Roles & Responsibilities
- Deliver personalized physical therapy services in patients' homes
- Develop and implement individualized treatment plans
- Assess patient progress and adjust care strategies accordingly
- Educate patients and families on exercises and safety techniques
- Collaborate with physicians and interdisciplinary care teams
- Document care accurately and maintain compliance standards
Qualifications
- Active California Physical Therapist license required
- BLS certification required
- Home health experience preferred but not mandatory
- Strong communication and interpersonal skills
- Ability to work independently in a home setting

- Therapeutic exercise and mobility training
- Home safety evaluation
