Roles & Responsibilities

• Provide direct patient care, including medication administration, wound care, and monitoring vital signs
• Collaborate with a multidisciplinary team to develop and implement individualized care plans
• Document patient assessments, interventions, and outcomes accurately and in a timely manner
• Educate patients and their families on health conditions and post-discharge care
• Advocate for patient safety and quality of care in a supportive environment
• Respond quickly to changes in patient condition and escalate as needed

Qualifications

• Active LPN or RN license in the state of Maine
• Current BLS certification (AHA or equivalent)
• Must have completed COVID-19 vaccination or have an approved religious/medical exemption
• Prior experience in a skilled nursing or long-term care setting preferred but not required
• Able to work 12-hour overnight shifts and pick up per diem assignments as needed

Knowledge & Skills

• Strong clinical assessment and critical thinking skills
• Knowledge of medication administration and infection control protocols
• Comfortable using electronic health records (EHR) systems
• Excellent communication and interpersonal skills
• Compassionate, patient-centered approach to care
• Ability to work independently and as part of a team
