Roles & Responsibilities

• Provide compassionate nursing care to residents in a long-term care facility
• Administer medications and treatments as prescribed
• Monitor vital signs and assess patient conditions
• Collaborate with interdisciplinary team to develop and implement care plans
• Document patient care accurately using PointClickCare EMR
• Float to different units within the building as needed

Qualifications

• Active LPN license in New York
• Current BLS certification
• Minimum 1 year of experience in a long-term care facility
• Travel experience preferred
• Valid driver's license
• NURSYS report with all active licenses

Education

• Completion of an accredited LPN program

Knowledge & Skills

• Proficient in PointClickCare EMR
• Strong clinical assessment and medication administration skills
• Knowledge of geriatric care and dementia care
• Excellent communication and teamwork abilities
• Ability to work block scheduling and every other weekend
