Roles & Responsibilities

• Provide compassionate bedside care and routine assessments for patients
• Administer medications and treatments safely and accurately
• Document patient status, interventions, and responses in the electronic chart
• Communicate effectively with RNs, providers, and the interdisciplinary team to support care plans
• Assist with patient education and discharge teaching that helps patients thrive
• Support a positive patient experience by demonstrating professionalism and empathy

Qualifications

• Active LPN license in New York (required)
• Current BLS/CPR certification (required)
• Practical nursing diploma or certificate from an accredited program
• Previous hands-on clinical experience preferred; per diem or long-term care experience a plus
• Ability to pass facility background checks and meet onboarding requirements

Education

• Practical Nursing diploma or equivalent (required)

Knowledge & Skills

• Solid clinical assessment and vital sign monitoring skills
• Medication administration and documentation best practices
• Comfortable using electronic medical records and charting systems
• Strong communication and teamwork in fast-paced environments
• Patient-centered mindset with attention to detail and safety
