Roles & Responsibilities

• Assess patient and caregiver needs to develop a comprehensive plan of care
• Serve as Case Manager, coordinating care across disciplines
• Complete accurate OASIS documentation in line with regulations
• Implement nursing interventions and high-tech skills like IV therapy and wound care
• Educate patients and families on medications, treatment options, and discharge instructions
• Collaborate with physicians, LPNs, and the interdisciplinary team for optimal outcomes
• Participate in peer review audits and case conferences to ensure quality
• Coordinate safe discharges and transfers, ensuring all paperwork is completed

Qualifications

• Active Registered Nurse (RN) license in Vermont or compact state
• Minimum 2 years of home health nursing experience
• OASIS certification or proven experience with OASIS documentation
• BLS certification required
• Strong clinical assessment and critical thinking skills
• Comfortable with high-tech skills such as IV maintenance, wound VAC, and complex wound care
• Excellent communication and time management abilities
• Valid driver's license and reliable transportation for home visits

Knowledge & Skills

• In-depth knowledge of home health regulations and evidence-based practice
• Proficiency in electronic medical records (EMR) and McKesson processes
• Understanding of Medicare and Vermont State Board of Nursing guidelines
• Ability to educate patients and families effectively
• Strong organizational skills to manage caseload and productivity standards
